Knilonat 200 mg (Nilotinib)

Knilonat 200 mg contains nilotinib, a second-generation BCR-ABL tyrosine kinase inhibitor (TKI) used for the treatment of Philadelphia chromosome–positive chronic myeloid leukemia (Ph+ CML). It is more potent and selective than imatinib and is used in chronic phase and accelerated phase CML where molecular response is required.

Knilonat 200 mg is a prescription-only targeted anticancer medicine and must be taken under supervision of a hematologist/oncologist with routine molecular monitoring (BCR-ABL PCR levels), ECGs and laboratory tests.

Product introduction

Nilotinib inhibits the BCR-ABL tyrosine kinase produced by the Philadelphia chromosome translocation t(9;22). This abnormal kinase drives uncontrolled proliferation of myeloid cells. By blocking this activity, nilotinib helps restore normal blood cell regulation, achieving deep molecular remission in many patients.

Nilotinib 200 mg capsules are a standard strength used twice daily in adults. Treatment is long-term and based on achieving molecular milestones. Monitoring includes BCR-ABL quantitative PCR, complete blood counts and ECG for QT interval prolongation.

Uses of Knilonat 200 mg (Nilotinib)

Knilonat 200 mg is prescribed for:

  • Philadelphia chromosome–positive chronic myeloid leukemia (CML) — chronic phase
  • Philadelphia chromosome–positive CML — accelerated phase (selected cases)
  • Patients resistant or intolerant to prior therapy with imatinib or other TKIs
  • Initial first-line therapy for Ph+ CML in adults (according to guidelines)

Note: Not indicated for blast crisis unless under specialist-directed protocols.

Benefits of Knilonat 200 mg (Nilotinib)

Clinical advantages include:

  • More potent than imatinib — achieves faster and deeper molecular responses
  • Effective against imatinib-resistant BCR-ABL mutations (excluding T315I)
  • Improves progression-free and overall survival in chronic-phase CML
  • Convenient oral therapy
  • Suitable for long-term maintenance of molecular remission

In Treatment of Worm infections

Important: Nilotinib does not treat parasitic or worm infections. This section is included solely for the required template structure.

Side effects of Knilonat 200 mg (Nilotinib)

Nilotinib has known adverse effects, especially involving metabolic parameters and the cardiovascular system.

Common side effects

  • Rash or itching
  • Headache
  • Nausea or abdominal pain
  • Fatigue
  • Joint or muscle pain
  • Hair thinning
  • Constipation or diarrhoea

Important risks: QT prolongation, sudden cardiac events, elevated liver enzymes, elevated pancreatic enzymes, hyperglycemia, lipid abnormalities, cytopenias, and fluid retention. Seek urgent care for chest pain, palpitations, fainting, severe fatigue, jaundice, shortness of breath or signs of infection.

How to use Knilonat 200 mg (Nilotinib)

  • Standard dose: 300 mg twice daily for newly diagnosed chronic-phase CML (using 200 mg capsules appropriately).
  • Alternatively: 400 mg twice daily for imatinib-resistant/intolerant patients.
  • Take doses approximately 12 hours apart.
  • Must be taken on an empty stomach — avoid food for 2 hours before and 1 hour after each dose.
  • Swallow capsules whole with water.
  • Do not skip doses or stop treatment unless your hematologist instructs.

Important: Taking nilotinib with food dramatically increases drug levels and can cause dangerous heart rhythm abnormalities.

How Knilonat 200 mg works

Nilotinib binds to and inhibits the ATP-binding pocket of the BCR-ABL oncoprotein, preventing phosphorylation and downstream signaling that drives leukemic cell growth. This restores normal myeloid cell maturation and reduces abnormal proliferation.

Safety advice

Heart conditions Nilotinib can prolong the QT interval — baseline and periodic ECGs recommended.
Food interaction Do NOT take with food. Food greatly increases absorption & risks serious cardiac events.
Pregnancy Not recommended; effective contraception required.
Breastfeeding Avoid during therapy due to unknown excretion and potential harm.
Liver disease Use cautiously; monitor LFTs frequently.
Drug interactions Avoid strong CYP3A4 inhibitors/inducers (e.g., azoles, rifampicin, grapefruit products).

What if you forget a dose?

  • If within a few hours of normal time — take the dose.
  • If more than 12 hours late — skip and resume the next scheduled dose.
  • Do not double dose.

All substitutes

Common alternatives based on CML guidelines:

  • Imatinib
  • Dasatinib
  • Bosutinib
  • Ponatinib (for T315I mutation)
  • Asciminib (newer BCR-ABL inhibitor)

Choice depends on mutation profile, tolerance, comorbidities and stage of CML.

Quick tips

  • Take on an empty stomach every time.
  • Avoid grapefruit products.
  • Keep consistent dosing times.
  • Attend all ECGs and PCR monitoring appointments.
  • Report chest pain, fainting or severe fatigue immediately.

Patient concerns

Will nilotinib cure CML? Nilotinib can achieve deep molecular remission, but most patients require long-term therapy. Some may qualify for treatment-free remission after sustained deep response.

How long before it starts working? Blood count improvement may appear within weeks. Molecular response is evaluated over months using BCR-ABL PCR testing.

User feedback

Patients report effective disease control with manageable side effects, though metabolic changes (glucose/lipids) and cardiovascular monitoring are common concerns. Long-term compliance is crucial for remission.
